Registering at your Practice
New registrations are being taken at all of our Medical Centres
To register, you need to attend one of our three practices.

How to Register:
You can call in at one of our practices at any time, but you may prefer to come during a quieter time, say after 11.00am and before 3.00pm, as you will need to fill in some registration forms and there may be a delay during busier periods.
If you are moving into the area or wanting to change GP’s from a local practice, you just need to bring your old medical card if you have one. You will be asked to fill in new registration form.
If you are registered elsewhere and need treatment and or staying in the area for a short time we may sign you up as a temporary resident. We will need contact details for your own doctor so that we can send details of your treatment to them to update your medical records.
If you are a visitor to this country and want treatment you will be asked to provide information about your country of origin and insurance status.
If you are from overseas and asking for permanent registration, you must produce your documents as proof of UK residency and gives you leave to reside in this country. If you are not able to produce the documents, we will not be able to register you.
If you are an asylum seeker, you need to attend the specialist surgery at Cape Hill Medical Centre, Raglan Road, Smethwick B66 3NR, or call 0121 558 2613.
To help speed up the process, you may wish to download the 2 registration forms (see below) and bring them into the practice with the required I.D.

If I've lost my medical card?
If you have lost your medical card you can get a replacement by telephoning the PCT where you were last registered and they will send you a new one by post, or you can call in and see them. There is a list of PCT’s on our website.
